    That's a few questions, now, isn't it?

    Italy became a nation under Garibaldi and Vitorio Emmanuel, wherein the poilital "shape" is the boot you see on maps. Before that, Italy was a bunch of non-aligned city-states, and Kingdoms.

    Its geographic shape was defined when the Mediterreanean Sea receded to what it is now, probably after the last Glaciation [40,000 plus years ago].

    Northern Italy, notably Milan and Genoa, were always places of trade, so fashions from around the known world were in evidence in the 15th century.

    Italians have known the shape of Italy for millenia. You don't know your a**ss from your elbow.
